This policy addresses the Peak Concierge Care PLLC informational website. It is separate from the practice’s Notice of Privacy Practices, which explains how patient health information is used and disclosed.

Information you choose to provide

If you contact the practice by email or phone, the practice receives the information you provide and uses it to respond to your inquiry and discuss membership. Do not send medical records, detailed symptoms, or other sensitive health information through ordinary email. Current patients should use the patient portal.

How this website works

This version does not include an inquiry database, advertising pixels, or analytics scripts added by the practice. Report previews and member-option interactions run in your browser. The hosting provider may process technical information, such as IP addresses and request logs, to deliver and protect the website. The website is designed for hosting on Netlify. Its technical logs are handled under the hosting provider’s policies.

External links and video

Links to the patient portal, Vimeo, Google, and other external websites take you to services with their own privacy policies. Embedded Vimeo players load only after you select a video. At that point, your browser connects to Vimeo, which may process technical information under its own privacy policy. Review the destination’s policies before sharing information.
